Our online application system makes it easy to enroll your child in our programs. Our application procedures are as follows:
New Weekender Application

Full payment is required for processing Weekender Application. This is a COMPLETELY REFUNDABLE FEE should your child be withdrawn by the Monday before the Weekender Program.
Payment can be made online by credit card during the application process or a check made out to “Summit Camp, LLC” is to be mailed to: 322 Route 46 West, Suite 210, Parsippany, NJ, 07054 USA.

After we receive your initial application, we will send you information on how to proceed with the process. Below is a list of the materials we will need from you in order to process your application.
Copies of one or more of the following recent reports (within three years):
Required Form:

Educator Report Form to be completed by your child’s Primary Teacher
At least one of the following, whichever are readily available:

Therapist report (if your child is in therapy currently)

Psychiatric Evaluation (if appropriate)
-
The social/emotional section of a recent IEP or Triennial review

Psychological or Neuropsychological evaluation

Upon receipt of the above materials, our Area Director will contact you to arrange a telephone interview. Subsequently, you or we may request a personal meeting as well.

Once our interview has been completed and we are all satisfied that your child will find success at camp, we will then provide you with the relevant enrollment materials.
Returning Weekender Application

Upon receipt of your application, our office will contact you to confirm enrollment.

As the session/s you have enrolled your child for approach, you will receive instructions on additional forms and/or information required.
If you have any questions, please do not hesitate to call your Area Director.
